Now that we know who you are, here are a few requests for filling out this form:

  • Please answer the questions by category, same as we report it in the journal. 
  • Please include information for changes in both the church buildings and parsonage, if appropriate.
  • Please group responses.  For example, if you renovated the church's sanctuary and narthex and nursery, please do not enter multiple responses, and instead submit this as one paragraph for the church, like this:
    "Painted sanctuary and installed laminate flooring; Painted narthex and replaced carpet; Painted nursery and installed two-part door to comply with safe-sanctuaries guidelines".
  • If in doubt, just put the information under "Question 10 - Other".
